What manual therapy actually is
Manual therapy is a set of hands-on techniques a physiotherapist uses to restore movement: joint mobilisation, soft tissue work, and specific stretches applied where testing shows the restriction actually is.
It is not a massage with a different name. The techniques are chosen after an assessment, applied to a specific joint or tissue, and immediately re-tested to see whether the movement changed.
When it helps
Stiffness that has outlasted the injury. A neck that will not rotate to one side. A shoulder that catches at a particular angle. A lower back that locks after sitting. Ribs and thoracic spine that stay stiff after a chest infection or long period of poor posture.
It is also the fastest way to make painful movement tolerable so that strengthening work becomes possible at all.
What it costs
Manual therapy is delivered inside a physical therapy session rather than billed as a separate service, because on its own it rarely holds. The session includes assessment, hands-on treatment and the exercises you take home.
Frequently asked questions
- Is manual therapy the same as chiropractic?
- No. Manual therapy is one component of a physiotherapy plan and is graded to what your tissue tolerates. It is combined with exercise, because a joint that moves better in the clinic goes back to being stiff if nothing changes the load on it.
- Will I hear a click?
- Sometimes, and it means very little. A joint that makes a noise is not more treated than one that does not. What matters is whether the movement and the symptoms changed when you are re-tested.
- Is it painful?
- It should be uncomfortable at most, and never more than you can breathe through. Tell the physiotherapist and the grade of the technique is adjusted.
- How many sessions before I notice a difference?
- Movement usually changes within the first session. Whether it holds is what the next two or three sessions tell you, and that is why the home exercises matter.
