Why the first steps hurt most
Overnight the foot rests in a relaxed position and the fascia shortens slightly. The first loading in the morning stretches sensitised tissue abruptly, which produces the sharp pain people describe so consistently that it is close to diagnostic on its own.
After a few minutes of walking the tissue adapts and symptoms ease, then often return later in the day after prolonged standing. Sitting for a while and then standing reproduces a smaller version of the morning pattern.
The heel spur is a red herring
Many people are shown an X-ray with a heel spur and told that is the cause. Spurs are found in a large share of people with no heel pain at all, and plenty of people with severe plantar fascia pain have no spur.
The spur is a response to load over time, not a spike stabbing into the tissue. Treatment is aimed at the fascia and the calf, and the spur is left alone.
Why the calf is half the problem
The plantar fascia and the Achilles tendon are functionally connected through the heel. A tight, weak calf changes how the foot loads at every single step, and it is the most consistently overlooked part of treatment.
That is why programmes built only on rolling a ball under the foot tend to produce temporary relief and no lasting change.
